What is an advance care directive kit

The South Australian Government developed an Advance Care Directive kit, which contains the form to be completed, instruction on how to complete it, who can witness and examples of statements that the person may choose to include to make their wishes known to health care providers and their substitute decision maker(s).

Why do we need an advance care directive?

Unexpected events like accidents, illness, or death can occur to anyone. A person may become incapacitated to make decisions for themselves or unable to express their wishes. 

Through an Advance Care Directive, a person can:

  • Appoint one or more Substitute Decision-Makers and/or

  • Write down their values and wishes to guide decisions about their future health care, end of life, living arrangements and other personal matters and/or

  • Write down health care you do not want in particular circumstances.
