Understanding your needs using one integrated assessment

After you register with My Aged Care, you may be referred to the Aged Care Assessment Service (also known as the Single Assessment Service). Australia now uses the Integrated Assessment Tool (IAT), which is used nationally by all assessors.  

This tool brings together the important areas of your health, daily life, home situation and goals into one consistent assessment — so you don’t need to repeat your story or undertake multiple assessments.

During the assessment, the assessor will work with you to:

  • Talk about what’s working well and what you’re finding difficult.

  • Look at how you manage everyday tasks.

  • Discuss your health, memory, mobility and safety.

  • Understand your preferences, routines, and what matters most to you.

  • Involve family, carers or support people if you choose.

  • Co-design a personalised support plan.

Using the integrated Assessment Tool, the assessor determines your level of need, and this helps My Aged Care understand:

  • How much support you require.

  • Whether you would benefit from entry-level support (CHSP) or higher-level Support at Home services.

  • What equipment, modifications, therapy or other supports may assist you.

  • Whether short-term or ongoing support will best meet your goals.

Your assessment may happen in your home, by phone or via video, depending on your circumstances and location. This nationally consistent approach means that no matter where you live in Australia, your assessment is fair, transparent and based on the same standards.
